客観的臨床能力試験(OSCE)の欠点の一つである妥当性の検証について、外来患者の満足度を用いた調査を行った。対象は平成13年度、14年度に佐賀医科大学総合外来で実習を行った6年次学生176名。学生の診療に対する満足度調査に回答の得られた外来患者は932名だった。満足度調査は米国内科専門医会が作成した患者満足度調査票(PSQ)を和訳して用いた。逆翻訳の結果、日本語PSQは米国内科専門医会に承認された。OSCEステーションは医療面接1、身体診察2、基本的検査1、カルテ記載1の5ステーション、各5分で構成した。PSQの平均スコアは3.56±0.66(満点5.0)で、平成11〜12年度に行っていた予備調査に比較して有意に向上していた。クロンバッハα係数も0.89で内的一貫性も保たれていた。OSCEのスコアは80.28±28.14(満点100)で、PSQとOSCEのスコアの相関は有意ではなかった。ただしOSCEスコアの低い群のPSQスコアは他の学生群と比較して有意に低かった。PSQスコアの向上は、佐賀医大のカリキュラム改善に起因すると思われた。OSCEとPSQスコアに相関がなかった原因は、ステーション数が少なく、学生の臨床能力を客観的に評価できていないなど、OSCE側の欠陥に起因すると思われ、今後の充実が必要である。ただし患者から低い評価しかもらえない能力の低い学生を検出する総括評価として用いるには妥当性があると思われた。
