まず、皮膚抗酸菌症のハンセン病の原因菌であるMycobacterium lepraeを特異的に検出するPCRプライマーについて、すでに我々が設定してあるプライマーの信頼性を多数の症例で再確認した。すなわち、全国の37施設からの依頼検査について検討し、臨床的に判断が困難な症例では特に診断的価値が高いことを実証した。また、M.lepraeを検出するプライマーについては、さらに感度と特異性の高いプライマーの設定が可能かどうかについて、同じ臨床検体を用いて比較検討することが今後の課題である。また、皮膚深在性真菌症であるスポロトリコーシスを検出するPCRプラィマーについても感度と特異性を検討中で、試験的な検討では良好な結果を得ている。その他Mycobacterium marinumについてもPCRプライマーの遺伝子配列の検討をおこなっており、並行して皮膚組織を臨床材料として処理方法の検討をおこなっている。皮膚組織の処理方法については、現在までの検討では、DNA抽出については機械的破砕に強力な蛋白変性剤を併用することが効果的であると考えられる。今後は病原体の活動性を遺伝子レベルで評価するため、皮膚組織からの効率的なmRNAの抽出法について検討を重ねる予定である。
