介護保険下のホームヘルプ業務は、介護報酬上の派遣区分により身体介護と家事援助に二分され、相談援助は基本サービスとして取り扱われている。また、家事援助を家事代行としてとらえる考え方があり、身体介護に比べ家事援助が介護報酬上低く評価されている。ホームヘルプ実践が身体介護・家事援助・相談援助の統合的援助であることを実証するためには、第1にホームヘルプにおける相談援助が身体介護や家事援助と統合的に提供されていること、第2に家事援助が利用者の生活を支える過程において、身体介護や相談援助と統合的に提供されていること、第3に身体介護が家事援助や相談援助と不可分の関係にあることを示す必要がある。昨年度は前二者の視点から研究を進め、今年度はそれらを分析し、ホームヘルプ実践の統合的援助として対応困難な場合が多いと予測できる身体介護を要する事例について、第1〜第3の視点でインタビューを実施した。(1)相談援助が身体介護や家事援助と統合的に提供されていることを示すための研究援助過程におけるホームヘルパーの働きかけと利用者との相互変化について、ホームヘルパー実践に関するインタビュー内容を修正版グラウンデッド・セオリー・アプローチにより分析し、(2)と関連させて検討した。(2)家事援助が身体介護や相談援助と統合的に提供されていることを示す研究家事援助効果が顕著にみられた事例における統合的援助についての検討結果を踏まえ、予防的側面においても分析を加え、統合的援助の意義を深めた。さらに、家事援助が有効である事例について、ホームヘルパーへのインタビューを実施し、詳細な統合的援助の過程をまとめた。(3)対応困難な身体介護を中心とした事例について、統合的援助が有効であったことを示す研究対応困難事例として挙げられることの多い内部障害者への事例について、利用者及びホームヘルパーにインタビューを実施し、統合的援助の意味と援助過程を示した。
