最近我々が開発した、原発性副甲状腺機能亢進症モデル動物であるトランスジェニックマウス(PTH-cyclin D1 transgenic mice ; PC mice)を用い、cyclin D1癌遺伝子によって発症する副甲状腺腫瘍が生化学的な副甲状腺機能亢進症を発症する機序について検討を行った。本マウスでは週齢が経つにつれ、あるときを境に副甲状腺過形成が急に加速し、血清Caが急上昇する傾向がある。これは、cyclin D1の副甲状腺における過剰発現による副甲状腺過形成がまず生じ、そして体細胞突然変異によりmonoclonalな腫瘍を形成し、著しい副甲状腺機能亢進症を呈することによると仮説している。まず、PC miceを帝王切開による微生物学的クリーニングによりSPF化し、その後コロニーを形成させた。カルシウム受容体作働薬であるcalcimimeticsのひとつであるKRN1493の、生化学的な副甲状腺機能亢進症抑制作用、副甲状腺細胞増殖抑制作用、そしてmonoclonalな腫瘍を形成抑制作用を検討するために、KRN1493の短期、長期投与試験を行った。短期投与試験として、47〜48週齢の雄性マウスを用い30,100mg/kgの1493投与試験を行った。血清Caは1493投与群で濃度依存的な低下が2時間、8時間で認められた。血清PTHは30mg/kg投与群で2時間後に低下が認められた。30mg/kg投与での血清Caの下がり値がWTマウスの血清Ca値と近く、長期投与試験の投与量は30mg/kg前後が妥当と推察された。また、薬効がどれくらい持続するかを確認するため、PTH経時変化を別途観察した。KRN1493投与量は30、100、1000mg/kg、投与後30min、1、2、8、24hrに採血し、血清CaとPTHを測定したところ、血清Caが最も低下するのは30mg/kgは2hr、100mg/kgは8hr、1000mg/kgは24hrと、投与量に依存して薬効が持続した。また、PTHは30mg/kgで2hr、100mg/kgで8hr、1000mg/kgで24hrまで低下したため、マウスではラットの約10倍の投与量が必要であるが、経口投与30mg/kgで充分効果が期待できると予想された。現在長期投与試験中であるが、連投後62日目の採血を行い、血清Caを測定したがまだ効果を認めることはできなかった(PC2媒体投与群:Ca9.866±0.170、PC2 1493 10mg/kg投与群:Ca 10.24±0.449、PC2 1493 30mg/kg投与群:Ca 9.851±0.176)。引き続き、投与を継続し、PCNA,BrdU等による増殖状態の評価等を行う予定である。
