間質性肺炎超早期の活動性評価をすべくWistar系雄ラット(平均。約350g)にブレオマイシン30mgを尾静脈より静注し間質性肺炎モデルラットを作製した。静注一週間後および二週間後にラットの気管切開部よりTc-99m DTPA aerosolを超音波ネブライザーを使用し両肺に吸着させた後ガンマカメラで後面より撮像を行った。得られた画像では気道沈着が顕著であり、肺野へのTc-99m DTPAの分布が認められなかった。このため肺野からのDTPA clearanceの評価が困難であった。病理組織標本では、肺うっ血、肺動脈血栓、血管炎を呈しており、これらの変化により二次性に気道狭窄が引き起こされたものと考えられた。理想的な間質性肺炎モデルの作製が困難であったため間質性肺炎患者における臨床的活動性とTc-99m DTPA aerosol scintigraphyとの関係を検討する事とした。対象は間質性肺炎患者24例(特発性12例、膠原病由来12例)で、全例にTc-99m DTPA aerosol scintigraphyを施行し、肺野に関心領域を囲み同部位の時間放射能曲線から半減時間T1/2を算出し、これをDTPA clearanceとした。DTPA clearanceと発熱、咳嗽、呼吸苦を中心とした臨床症状とを比較検討したところ、臨床症状の強い患者群の平均DTPA clearanceは22.9分、症状の乏しい患者群は28.9分と臨床症状の強い患者群のDTPA clearanceは有意に亢進していた。特にDTPA clearanceが25分以下では間質性肺炎の臨床症状からみた活動性の正診率が高く診断に有用であった。以上より間質性肺炎の活動性評価にTc-99m DTPA aerosol scintigraphyは有用であると思われた。
