MRIによる乳癌乳管内進展の診断基準を確立するため、乳房切除術にて得られた標本との比較を行った。これまで使用したMRI画像は再構成画像およびMIP画像であった。どちらも病理組織の厳密な比較が困難であった。そこで病理組織の割面に相当すると考えられるpartial MIP画像がもっとも適していると考え、病理組織所見との対比に使用した。また全乳腺の高分解能Dynamic MRI撮像を行い、病変の増強パターンも検討項目とした。平成16年2月から12月までに乳房切除術が行われた33例中、乳管内進展を伴い、病理組織標本の変形が軽度で、良好なMRI画像を得ることができた10症例を対象とした。切除された乳腺組織は腫瘍の中心と乳頭を結ぶ線にて割を入れ、病理組織標本を作製した。検鏡により癌組織の広がり、乳腺症などの病変を決定した。全例にてMRIでの乳癌病変の描出は良好であった。また、全例にて乳管内進展の指摘が可能であった。乳管内進展の範囲についてはMRI画像と病理組織標本がほぼ一致したものは5例、過少評価は3例、過大評価は2例であった。過少評価の原因としては、平坦型(Flat type)乳管内癌の描出がMRIでは困難であったため、過大評価の原因としては、MRIにて乳頭に連続する線状構造として描出された正常乳管を乳管内進展と判断したためであった。主病変や乳管内進展以外にMRIにて小結節として描出され、病理学的に判断可能であった病変は23病変であった。乳腺症が22病変であり、乳癌が1病変であった。Dynamic curveは乳腺症22病変中2病変にmalignant patternを示すものがあった。乳癌1病変のDynamic curveはmalignant patternを呈していた。全乳腺の高分解能Dynamic MRI撮像は、乳管内進展の描出や乳腺症との鑑別診断に有用と考えられた。

MRI was used to determine the baseline level of breast cancer in breast duct, and the standard of mastectomy was improved. The MRI portrait is then reconstructed into a portrait. The MIP portrait is reconstructed. The density of the pathological tissue was much higher than that of the pathological tissue. The cut surface of the pathological tissue is quite different from the partial MIP portrait. The image of the pathological tissue is much higher than that seen in the pathological tissue. The high resolution of whole mammary gland can be used to image the whole breast by Dynamic MRI, and the disease should be enhanced. Among the 33 cases of mastectomy in Pingcheng in February and December of 16 years, there were 33 cases of mastectomy, intraductal progression, histopathological markers, and good MRI portraits. After excision, the breast tissue was cut into the breast tissue center, and the pathological tissue marker was used as a guide. The cancer tissue, breast cancer, breast cancer, breast cancer and breast cancer were determined. The whole case of MRI breast cancer showed a good picture. In all cases, the progress of the mammary duct may be difficult to understand. In the breast duct, there were 5 cases of concordant dysplasia, 3 cases of hypothyroidism and 2 cases of excessive dysphagia in the pathological tissue of MRI images. The causes of intraductal carcinoma in the breast duct (Flat type) showed that the breast ducts were normal, the normal breast ducts were normal, and the breast ducts were normal. Outside the breast duct of the main disease, the small results of MRI examination showed that the diagnosis of pathology and pathology might lead to the disease of 23 diseases. Breast cancer 1 is characterized by Dynamic curve malignant pattern. High resolution of the whole breast can be used for Dynamic MRI imaging and intraductal progression to identify the diagnosis of breast disease.
