調査により得られた患者アウトカムデータにおける医療効果測定指標の信類性・妥当性の検討および精緻化を行った。測定尺度の因子構造の概略を探るために、得られた患者満足項目群における既存測定各領域に位置づけられた各項目を用いて探索的因子分析を行った。主因子法による因子抽出を行いKaiserの正規化を伴うoblimin回転の結果、各因子に対する各項目の明瞭な負荷が得られ、因子間でcross loadも殆どみられず非常に良好な結果を得ることができ、結果的に、約8領域に縮約されることが判明した。このため、暫定的な因子モデルの構造と安定度を解明するために、各領域を潜在変数とする確認的因子分析による共分散構造分析を行った。その結果、モデルとデータの不一致測度であるCMIN/DF値は14.842と若干、大きめの数字が観察されたが、モデル適合度を表す他の基準指標(NFI、CFI、RMSEA等)は基準値を十分上回っており、適合度の高さを示す満足すべき結果が得られたものと考えられる。また、本調査データおよび再調査データ共に、各測定領域内のCronbachのα係数は、それぞれ、0.798〜0.954および0.856〜0.953と極めて高い内部一貫性を示す結果となった。さらに、各測定領域間のtest-retest reliabilityを表すPearsonの積率相関係数は、各測定領域間で0.668〜0.876を示しており、高い信頼性も備えていると考えられる。これらの測定指標を用いて、患者の重症度や健康関連QOLを補正(ケースミックス補正)した上で病院間比較を試みたところ、病院間での測定値の散らばりは比較的大きく、施設間での判別および収束妥当性もある程度、具備していることを示唆する結果となった。
