濾胞性リンパ腫は緩慢な自然史を特徴とするリンパ腫である。進行期症例に対しては化学療法が選択されるが、治療強度を上げても生存延長には寄与しないことが報告されてきたが、近年濾胞性リンパ腫の寛解維持、無病生存の延長を目指して様々な造血幹細胞移植が試みられ、またキメラ型CD20抗体が開発され優れた奏功性を有することが報告されるようになった。濾胞性リンパ腫ではbcl-2/IgH PCRを用いることで微少残存病変(MRD)の評価が可能であり、今後骨髄移植症例やキメラ抗CD20抗体においてMRDモニタリングが有用と考えられる。まず、bcl-2/IgH PCRにおいて定性PCRの検討を行った、9症例の濾胞性リンパ腫において検討したが、PCR陽性バンドが得られた症例は一例のみであった。そこで我々は以前よりリンパ腫のMRDについてIgH再構成遺伝子を用いた症例特異的PCRについて検討を行っているが、濾胞性リンパ腫においてMRDの検討を行った。濾胞性リンパ腫10症例において化学療法を含めた治療前より末梢血・骨髄血MRDモニタリングを行った。2症例でMRD持続陽性を認め、そのうち1例で再発を認めた。そこでMRD持続陽性2例においてはTaqman probeを用いた定量PCRの検討を行った。再発を認めない1例では、定量PCRは検査感度以下であったが、再発した1例は、診断時末梢血(PB)/骨髄血(BM)(ともに検出感度以下:診断時リンパ節を10°とした)治療後PB/BM(検出感度以下/7.37x10^<-3>)再発時PB/BM(1.51x10^<-2>/3.31x10^<-2>)であった。定量は定性検査と比較し感度は劣るものの定量高値については再発のリスクが考えられモニターに有用と考えられる。しかしながらさらなる観察期間、症例における検証が必要と思われた。
