1.評価指標の検討のための調査A保健センターの高血圧・高脂血症・糖尿病の個別健康教育の参加者の個別健康教育の参加による変化についてのグループ討議の内容から、逐語録を作成し、その内容を抽出した。その結果、運動や食事の内容や量の変化など「行動の変化」、体重や血液データなど「測定値の変化」、行動変容、行動継続への意識、行動への関心など「意識の変化」、家族や仲間の協力など「周囲の協力」が抽出された。同様に、個別健康教育を行った保健師に対し、対象者の変化についてグループインタビューを行い、逐語録を作成し、個別健康教育の参加による変化についての内容を抽出した。その結果、行動変容への意識、行動の継続化、行動への自信など「意識の変化」、体重や血液データなど「測定データの変化」、食事や運動の行動の「行動の変化」、家族の行動の変化など「周囲の変化」が抽出された。2.個別健康教育のプログラムの作成および実施糖尿病を対象とした個別教育プログラムを作成した。それは、自己観察、行動目標の設定、目標の自己評価の内容を含む、16週間のプログラムであった。評価指標は、「行動の変化」として食事と運動の自己管理行動、「測定値の変化」としてHbAlc、「意識の変化」として食事と運動の自己効力感を取り上げた。地方都市にあるB大学病院医学部付属病院とC総合病院に通院する糖尿病患者23名を対象に個別教育プログラムを実施した。個別教育プログラムの修了者は12名(男性6名、女性6名)(終了率52.2%)、平均の年齢は60.5±9.2歳、罹病期間は9.1±5.3年であった。糖尿病の治療方法は食事と運動療法のみ3名、内服療法7名、インスリン注射療法2名であった。平均のBMIは21.6±2.8、血糖値は143.5±34.8mg/dl、HbAlcは6.7±1.1%であった。プログラム前後の比較から、食事(p<0.05)と運動(p<0.05)の自己管理行動得点がプログラム前より後で有意に上がったが、自己効力感とHbAlcに有意な差は認められなかった。自己管理行動の有意な変化は、プログラムによる効果であると考える。また、自己効力感とHbAlcで変化の認められなかった原因は、対象者のコントロールが良好であったため、効果が得られにくかった可能性が考えられる。
