造血幹細胞移植前・移植後の時期において、造血器腫瘍患者の心理状態を明らかにしたうえで看護援助の検討を行うことを目的とし、急性リンパ性白血病の患者6名に移植前・移植後のインタビュー調査を実施した。インタビュー内容は、移植前には、病名告知時の気持ち、移植に対する思いについて、移植後には、移植を受けた後の気持ちや移植後の移植に対する印象の変化、医療者に対する思いなどの内容を含む半構成的インタビューガイドを用いて行った。分析は、逐語記録を意味内容ごとに類型化し整理していき、文脈ごとに気持ちを表すタイトル名をつけた。その結果、移植後の気持ちの構成要素として,10サブカテゴリー,6コアカテゴリーを抽出した。コアカテゴリーは、【命懸けで受けた移植後も払拭できない再発への恐怖】、【病とともに生きる覚悟と家族への思い】、【ドナーへの感謝だけではすまない気持ち】、【治療・看護システム整備への要望】、【治癒し家族と暮らすことへの希望】、【残りの人生を役立てたいという希望】と命名した。昨年度報告した移植前の気持ちの構成要素の結果ならびに本年度の結果から、造血器腫瘍患者の造血幹細胞移植前後の心理過程として以下のことが明らかになった。患者は白血病に対する恐怖心に加え、命懸けで受けた移植後も再発への恐怖が拭いきれずにいること、その一方で治癒し家族と暮らすことへの希望を持っていること、また、先端医療である移植治療・看護システム整備への要望を持っていることが明らかになった。したがって、造血幹細胞移植を受ける造血幹腫瘍患者に必要な看護援助としては、移植に関する情報提供システムの整備と、移植決定前から移植後の生活を想定し、患者が納得した上で治療を意思決定できる看護ケアシステムの構築が必要と考えられた。
