産褥早期における新生児に対する母親の応答性を明らかにするために、13名の母子に対して産褥4日目に授乳場面のビデオ撮影を行った。母子のsensitivityの評価にはPrice(1983)のAssessment Mother Infant Sensitivity Scale(AMIS)を用いた。また新生児の状態と母親の応答反応との関係を明らかにするため、AMIS得点の高い事例と低い事例に対してイベントレコードを作成し時系列的に分析を行った。AMISは産後3ヶ月以内の授乳場面における母子のsensitivityの評価尺度で、母親項目15・児項目7・二者関係項目3の3下位尺度で構成され、各項目1〜5点で評定される。日本語版は研究者らが作成し、産褥早期における信頼性と妥当性を検討したものを使用した。(AMIS日本語版の内的整合性:母親項目α=0.85、児項目α=0.61、二者関係項目α=0.26)二者関係は産褥早期に関してはまだ検討の余地があるため今回分析対象から外した。3名による評定者間一致率(KendallのW)は母親項目W=0.77(P<0.01)、児項目W=0.85(P<0.01)である。対象のAMISの平均得点は母親項目54.3±5.2、児項目19.1±4.0であった。1)AMIS得点の高い事例と低い事例を時系列的に分析すると、児に対する注視と声かけの頻度・生起時間に違いが見られた。また得点の高い母親の応答は児の状態変化に対して連鎖していた。2)AMIS得点の低い母親の児には得点が低いケースと高いケースの両方が見られた。前者は児がまどろみがちで母親の働きかけは一方的かあるいは少ない状態が観察された。後者では母親の関心が児の状態に向いておらず、より養育的にハイリスクな状態であると考えられ、母親の応答性を妨げている要因について明らかにしていく必要があると考えられた。
