项目摘要

この研究は、親と暮している重度の身体障害を持つ人の中で、介助者の介助を受けながら住みなれた地域で1人で暮らすという地域生活形態を選んだ人たちが、それを実現する過程に焦点をあて、その実現を支援する社会福祉実践の方法を、身体障害者通所授産施設・青葉園におけるフィールドワークをもとに明らかにすることを目的に行った。重度身体障害を持つ人が1人暮らしを実現するためには、その人ら自身による「生のひきうけ」が必要である。そして生のひきうけに向けて、青葉園はじめ複数の関係機関のネットワークによる支援が展開される。まず、障害者が普段通所している青葉園を起点に、関連機関による支援ネットワークが形成される。そして、支援ネットワーク内の機関による役割分担が行われ、各機関による支援が提供される。各機関の役割は生のひきうけ過程に応じて変更され、あるいは新たな機関が加わったり抜けたりしてまた新たな役割が分担され、ネットワークによる支援は続いていく。通所施設・青葉園は支援ネットワークの起点であり中核の1つでもあるが、大別すると次の2つによって、障害者の生のひきうけを支援する:(a)障害者自身がエンパワーメントするために伴走すること、(b)環境と障害者を媒介すること。(a)は、障害者自身の変化に対応する力や決定・判断する力を向上させるために伴走することであり、その具体的な方法として、(1)障害者とその状況についてのアセスメント、(2)コミュニケーション支援、(3)障害者の判断・決定・問題解決の支援、(4)障害者や他の社会資源の役割の補完・代替、(5)障害者の迷いや不安、葛藤に添う、(6)アドボカシー、の6つが見つかった。(b)は、障害者とその環境の間を媒介することであり、具体的には、(1)障害者と家族のマッチング、(2)介助者:養成と資源化、(3)他機関:協働と資源化から構成される。
