项目摘要

先進国各国において拡張する公的医療支出の問題を解決するために提案されている医療貯蓄会計に関して、その最適な仕組みを定性的に解明し、かつその医療貯蓄会計制度を効率性と公平性の両面から定量的に明らかにすることが、この研究の最終的な目標である。そのための第一ステップとし、この若手研究Bでは、最適な医療貯蓄会計の定性的な仕組みを明らかにし、また家計内のリスクの配分がどのように行われているか実証的に明らかにすることを目標としていた。定性的な仕組みに関しては、多くのことがこの研究によって解明された。とくにモラルハザードが存在する場合にどのような政策が経済的な最適条件を満たすために必要かを解明した。この結論は、この分野での国際的なトップジャーナルであるJournal of Public Economicsに出版されることになった。定量的な解明に関しては、家計内のリスク配分の仕組みを実証的に検証する必要性があるという認識から、家計内の資源配分に関して3つの仮説をたてそれを検証した。具体的は、単一家計モデル、非協力ゲーム均衡モデル、バーゲニングモデルの3つの仮説を、日本の家計データに関するパネルデータを使い検証し、この実証結果を大阪大学大学院生の山田憲君と共同でTCERマクロカンファレンスで発表した。
