项目摘要

(1)遠隔医療導入のための小児救急医療の現状評価に関する分析地域住民の視点から小児の救急医療体制において適切な施設の数と規模を明らかにするために、階層分析法(AHP)により地域住民に対する意識を分析した。AHPは3層とし、評価層には「総合病院」「高度の医療設備がある」「待ち時間が短い」など7項目、代替案は医療機関の規模とし「小規模」「中規模」「大規模」の3案とした。調査対象は、北海道岩見沢市および南富良野町の幼稚園・保育園に通園する子供を持つ親、それぞれ100名、50名とした。回答率は50%強であった。岩見沢市の15名の分析結果では、「小児科医のいる施設」の評価値が最も高く0.38、次いで「病院の評判が良い」が0.16であった。(2)小児救急医療を代替するための遠隔医療システムの経済性に関する分析地域住民の視点から小児の救急医療体制を代替する遠隔医療システムの経済性を分析するために仮想評価法(CVM)によるWTP(支払意思額)の算出を試みた。本調査では遠隔医療は患者の診察時に医師が遠隔地にいる医師と通信を行うこと(Doctor to Doctor)を想定し、小児の急病患者の受診時に診察を行う医師が小児科または疾患に対応する専門医と動画像等を用いてコミュニケーションが取れるシステムを仮定した。対象は北海道岩見沢市および南富良野町の幼稚園・保育園に通園する子供を持つ親、それぞれ300名、50名とした。回答率は50%強であった。42人のWTPは4,156円であった。この結果は、今後の遠隔医療の普及を計る上で基礎資料になるものと考えられる。
