(1)心療内科に紹介される癌患者と「精神医学的診断なし」の患者群の探索を以下の方法で行なった。【方法】H15年4/1-H16年1/31までに呼吸器専門病院心療内科に紹介された癌患者について、厚労省MF内富班のデータベース作成の共同研究手法を用いて、個人情報が特定できない観察研究を行なった。【結果】依頼患者102名。男/女72/30。平均年齢65.1歳。原発件肺癌92、転移件肺癌2、悪性中皮腫3、その他5。病期は1/2/3/4:6(7%)/4(4%)/14(15%)/68(74%)。PS;0/1/2/3/4:5(4%)/30(29%)/23(22%)/21(20%)/23(22%)。精神医学的診断(DSM-4);診断なし/適応障害/大うつ病/せん妄/その他:39(39%)/24(24%)/14(14%)/19(19%)/4(4%)。【考察】診断なし群の頻度が同定された。同群の心療内科受療の準備性に関する追研究を行なう予定である。(2)肺癌患者において、外来化学療法移行に関する恩恵と負担に関する行動科学的研究をTranstheoretical Modeを用いて探索的な検討をおこなった。【方法】研究参加の同意が文書で得られた、肺癌患者22名を対象とし、外来化学療法への移行に関する準備性、恩恵と負担に関する質問を半構造化面接にて行なった。会話はテキストデータ化され、意味単位に分割し、その内容を独立したコーダーによって判定された。【結果】準備性は前熟考期7名;熟考期6名;準備期3名;実行維持期6名であった。内容分析は恩恵19カテゴリーと負担19カテゴリーが得られた。【考察】外来化学療法移行の準備性の妥当な段階評価と、恩恵、負担要因の内容が明らかになった。身体心理社会的因子を含む量的研究を追加し、外来化学療法移行の準備性に関する要因と心療内科的援助法を明らかにする予定である。

A follow-up study on the preparedness of patients in the same group of cardiologists (2)Lung cancer patients, foreign chemotherapy related to the burden of mobile science research, use of medical research [Methods] The study involved 22 lung cancer patients with consent documents and semi-structured interview questions about the relationship between the preparation and burden of exogenous chemotherapy. The conversation is divided into two parts, meaning that the content is independent. [Results] 7 students in pre-maturity test period, 6 students in maturity test period, 3 students in preparation period and 6 students in maintenance period. Content analysis: 19. [Investigation] The preparation for the migration of exogenous chemotherapy is appropriate. The evaluation of the stage, the grace and the burden are important. Additional research on psychosocial factors, preparation for the transition of exogenous chemotherapy, and identification of cardiac support methods
