【目的】高齢者社会の到来に伴い在宅要介護者に対する歯科医療の対応は大きな課題であり、歯科医療のニーズが増加しており、医療の対応および問題点の把握は重要と思われる。しかしながら在宅要介護者の口腔健康状態と身体的機能の回復の程度を総合的に評価した報告は十分ではない。今回在宅要介護者の口腔健康状態と身体的機能の関連性を検討するため以下の調査を行った。【方法】調査対象は、歯科検診・歯科治療に協力可能な岩手県大野村の在宅要介護者15名(平均年齢71.0歳、男性10名女性5名)とした。全身状態に関する項目では、基礎疾患、障害区分を調査し、日常生活機能の評価(ADL評価)についてアンケート調査を行った。口腔内診査は、総残存歯数、Eichner分類による咬合支持域、歯周疾患や齲蝕疾患、補綴状況について診査した。【結果と考察】1.宅要介護者となる疾患の既往は、脳血管障害が80%と最も高く、その後遺症である肢体不自由が障害区分として最も多かった。現有疾患としては高血圧(30%)、腰部の障害や糖尿病などの代謝疾患(各20%)であった。2.ADL評価は、アンケートの回答を数量化し、3段階に区分した。総合的ADLの度数分布は点数の高い(自立が高い)順から10%、60%、30%であった。総合的ADLと基本的ADLの度数分布は類似していたが、手段的ADLの度数分布は低い方に、また摂食に関するADLの度数分布は高い方にそれぞれ位置していた。3.主訴を有する在宅者は47%で、義歯に関する項目が多かった。何らかの処置を要する割合は73%を占め、主訴の有無と処置の必要性は必ずしも一致しなかった。全身状態の低下とともに、口腔健康管理に対する本人および介護者の意識の低下が反映されたものと推測された。4.在宅者の平均残存歯数は6.8本で、咬合支持域のないEichnerC区分が80%を占めた。一方A区分となった在宅者は13.3%で全て欠損歯のない有歯顎者であった。

[Objective] The arrival of a high-level society is accompanied by a major issue of dental care, an increase in dental care, and an important consideration of problems. A comprehensive assessment of the oral health status and the degree of recovery of body function of the home care provider is presented. This paper discusses the relationship between oral health status and body function in primary caregivers. [Methods] Fifteen residents (average age 71.0 years, 10 males and 5 females) in Ono Village, Iwate Prefecture were investigated for possible collaboration in dental examination and treatment. Items related to general health include surveys on basic diseases and impairment differentiation, and surveys on the evaluation of daily life functions (ADL evaluation).
