社会参加に難渋する外見からは障害が判り難い在宅脳外傷者は適切な支援体制が極めて不十分な状態であり、実効的な支援がないまま家族に負担を強いている。その様な状況の中、脳外傷者の家族の互助的な社会的活動が自発的に起こり脳外傷当事者組織が運営する作業所が開所され出した。しかし、当事者である在宅脳外傷者と家族が当事者作業所にどのような期待を寄せ、何が得られているかは不明である。そこで、当事者ならびに作業所代表者の当事者作業所への思いを知ること、現状と拡充すべき当事者作業所の機能を明らかにする事を通して、在宅脳外傷者の社会適応の実態を把握し、脳外傷リハビリテーションの介入指針や脳外傷者とその受け皿となる地域社会双方への介入についての基礎的な知見を得る事を目的として研究を実施した。同意の得られた9組の在宅脳外傷者と家族に半構成的面接を、10ヶ所の作業所代表者に質問紙による調査を実施した。その結果、当事者が作業所に懐いている11の期待を明らかにした。つまり、「成長の促しの場」、「個別に応じた支援の場」、「居場所」、「障害の本質を知る場」、「要素的な障害への介入の場」、「自分探しの場」、「社会的基礎技能の習得の場」、「心理的な安寧の場」、「情報収集の場」、「高次脳機能障害全般に関する窓口の場」、「家族が助け合う場」を求めていた。また、現状では「心理的な安寧の場」、「生活のリズムを確立する場」、「居場所」、「社会的マナーの習得の場」を当事者作業所で獲得していた。更に、「個別に応じた段階的な支援の場」、「次の目標に向けた準備が行える場」、「できる事を探索し発見する場」を今後に期待していた。代表者は、「成長の促しの場」、「居場所」、「家族が助け合う場」として機能する事を望み、「作業所の運営と活動の充実に向けた取り組みの場」が拡充すべき機能であった。以上、殆ど研究がなされていない脳外傷当事者組織が運営する作業所関係者の期待と機能していること、これからの展開が望まれることを明らかにし、今後更に研究を進める分野の構造を示した。
