项目摘要

医療事故の顕在化に伴う安全と患者の信頼の確保、在院日数の短縮化、関連機関との連携推進のために相談窓口の設置がされ、患者の相談に耳を傾け、サービスの質の向上のため活用することが重要視されている。医療サービスにおける情報は、非対称性と不確実性が強い。医療従事者の患者への情報提供は慎重であることが多く、医療広告の緩和、医療内容の開示などで配慮されている。病院のHPで看護師はどのような情報を提供しているか、近畿圏内の日本医療機能評価機構の認定をうけた病院283病院について詳細に分析した。看護独自の頁を掲載している病院が全体の52.2%であり、そのうち、看護部の目標、紹介100%、継続教育69.7%、看護体制46.6%、委員会活動39.2%であり、人事考課12.4%など多岐にわたり掲載していた。HPは患者だけではなく、就職希望の看護師への広告であるともいえるが、継続教育、人数、委員会活動を明示することでどのような体制で看護を提供しているか明確にしているとも理解することができる。入院案内の中で、看護師体制を明記する病院73.8%、相談窓口の設置51.4%、その他満足度調査、事故報告書、web医療相談、病気や治療についての情報などが掲載がされていた。看護師のサービス評価の認識についてのアンケートを作成し、全国の病院のうち無作為抽出した200病院の看護管理者あて配布した(有効回答数12%)。サービスの評価として利用しているものは、看護ケアの際の患者の反応83%、患者満足度調査75.0%、相談窓口に寄せられる意見54.2%などであった。これらの評価は、患者の病状や医療者への信頼感など、様々な要素が絡み合っているため、敏感になりすぎず活かしていきたいという認識であった。また、系統だって蓄積、分析するシステムをすでに整備している病院は一部であり、今後検討していきたい課題であると回答していた。
