项目摘要

本研究は、ドレフェス・モデル(Dreyfus&Dreyfus、1981年):"Novice(看護学生)","Advanced Beginner(経験1年未満の看護師)","Competent(臨床経験2〜3年)","Proficient(臨床経験4〜6年)",and"Expert(臨床経験7年以上)"を適用し、ナースへのインタビューデータをもとに内容分析を行った以下に結果を示す。<日本の看護師の実践する「安楽」の内容(属性)>1)Advanced Beginner:ベッド上での体位保持2)Competent:ベッド上での体位保持、ベッドメイキング3)Proficient:清潔の保持(全身清拭、口腔ケア、シャンプー、髭剃り)、ペインコントロール、社会資源の活用、スピリチュアルケア、睡眠への援助4)Expert:褥瘡ケア、個人的ニードに応える、リラクゼーション(ボディマッサージ)、ターミナルケア、患者移動、ペインコントロール、本研究結果は、どのような臨床経験を積んだ看護師であっても、「安楽」なケアを実践していることを示している。"Advanced Beginner(経験1年未満の看護師)","Competent(臨床経験2〜3年)"の実践している「安楽」なケアの内容(属性)は、身体的状況に関することのみであった。だが、経験を積むにつれ、属性は複雑になり、身体的側面と精神的側面を含むようになった。本研究の結果は、基本的な看護の概念の知識となり得、また、病院での安楽なケアの実践の促進を推奨するものである。
