15年度の文献検討より、周手術期実習において手術・麻酔侵襲により急性状況にある患者の身体的変化の理解、および術後急性期にある患者の観察項目の理解について特に基礎看護教育課程の学生は困難を抱えていることが明確になった。これより今年度本研究では、手術侵襲・麻酔侵襲による生体反応、および周手術期のアセスメント・看護の要点の学習を目的とした学習教材の構成(コースウェアフローチャート)および各フレームを作成した。教材の具体的教授内容は、(1)手術侵襲のタイプ、(2)手術侵襲からの回復過程を示したMooreの4分類、(3)侵襲に対する神経・内分泌系反応、(4)侵襲に対する代謝系反応、(5)侵襲に対するサイトカインの働き、(6)術後急性期のアセスメントポイント、(7)術後急性期の看護のポイント、の計7項目であり、モデルによる手術後の患者状況設定映像を含む画像を数多く取り入れることで、理解を促すよう配慮した。教材は各項目の質問と解説、および誤答ごとのコメント表示を挿入した解説分岐が含まれ、全40フレーム余りで構成されている。教材は今後の汎用性を考慮し、html形式で作成した。Html形式は教材利用者が学習項目を随時選択して実施することが可能であり、また前後のフレームに自在に行き来できることが利点であるが、一方で学習履歴の分析が困難であり、この点については今後の課題となっている。CAI教材を看護基礎教育に導入している岐阜大学医学部看護学科視察より、学生が実習前に手術に関連したCAI教材を導入し自己学習することで、手術室入室オリエンテーションにおいても人的・時間的資源の削減につながるという効果の実践報告を得た。このことから、今後本研究で作成した教材の学習効果を検証するとともに、e-learningを視野に入れた多機関での教材共同開発・教材共有を行うことで、基礎教育過程の教育効果に寄与するものと考える。
