我が国では現在、自殺死亡者が1998年以降、3万人を超えており、大きな社会問題となっているが、自殺を考えたり、試みる人がどの程度いるのか、また、その背景にあるうつ病やストレスによって引き起こされると考えられる精神疾患の有病率などの現状について、ほとんど明らかにされていない。本研究では、統合失調症を除く精神疾患の頻度や関連要因などについて検討することを目的とする。本研究は自治医科大学の倫理審査委員会での承認も受けたのち、栃木県佐野市において、住民基本台帳を利用し、調査対象地区の約3400世帯の20歳以上の住民7169人から調査対象者1041人を無作為抽出し(抽出率14.5%)、WHOの標準化された方式に則って訪問面接調査を行った。調査は、対象者の調査への同意を確認したのち、コンピューターと所定の調査用ソフトウェアを用いて行い、回答内容はその場入力をした。抽出総数1041人から対象外に該当する111人を除いた中で、550人(男251人、女299人)に対し面接調査を実施し終了した(調査への協力率は59.1%)。年齢構成は男20歳代6%(16人)、30歳代12%(30人)、40歳代20%(50人)、50歳代22%(55人)、60歳代21%(53人)、70歳代以上19%(47人)、女20歳代9%(26人)、30歳代16%(48人)、40歳代16%(49人)、50歳代21%(62人)、60歳代18%(53人)、70歳代以上20%(61人)であった。自殺行動をとったことのある割合では「自殺を試みた」が男1%(2人)女2%(6人)、「自殺の計画を立てた」が男1%(3人)女2%(5人)、「真剣に自殺を考えた」が男6%(16人)女9%(28人)、不満、悩み、苦労、ストレスの程度では「大いにあった」と回答した者は男13%(32人)女14%(42人)、自分自身の精神的健康について「不健康である」と回答した者は、男5%(12人)女6%(18人)であった。
