本研究は、生活モデル理論に基づくプログラムを実施している高次脳機能障害者の小規模作業所での実践を評価することで、生活モデル理論によるプログラムの有効性を実証し、高次脳機能障害者にとってより効果的な生活支援プログラムを開発することが目的である。昨年度末から今年度は、これまでの生活モデル理論に基づく高次脳機能障害者の生活支援プログラム評価に関する研究の成果を踏まえ、プログラムを途中で放棄(作業所を長期間休むなど)した利用者への支援と、就労支援のプログラムの評価を試みた。リハビリテーションプログラムを放棄する者は(1)通所開始間もない頃に「自分は(健常者であるので)他の障害者と一緒にされたくない」と、障害が認知できないことから早期にプログラムから離脱するケースと、(2)長期間リハビリテーションに参加し回復もみられるが、他の障害者が一般就労したといった情報や、家族等から回復を急がされる(「ずいぶんよくなってきたから、そろそろ就職できるんじゃないの」といった)言葉により、簡単に自分にとって必要なプログラムを見失うことが見受けられた。(1)の場合は、無理に通所開始を促すのではなく、自分なりに就労や復学をした結果やはりうまくいかないことを理解するまで待っしかない。しかし、(2)のケースでは、ある程度様々な状況や自分自身の障害も受け入れ理解することができるようになるため、今どのようなことで混乱し、何があるから慌てているのか、自分にとって何が必要なのか、今まで成功したことと失敗したことはどんな違いがあるかについて、繰り返し面接を行うことで、落ち着きを取り戻しプログラムに参加できるようになる。ただし、この面接を通じた支援が途切れてしまうとかなり長期間プログラムから離脱し、結果として混乱した状況も長期化する。電話などでも支援を行うが、直接面接し支援を行うケースよりもプログラムへの復帰率は低い。今後も引き続き、プログラムを長期離脱した利用者への支援のあり方を検討する必要がある。就労支援については、事例が1例みられたが、開始直後であるため評価には至っていない。しかし、復職に向けジョブコーチが復職先の上司と打ち合わせを繰り返し、試験的に受け入れてもらうなど進展は見られる。継続して就労支援プログラムについても研究する必要がある。
