口腔ケアが胃癌発生に関与するか検討するため、当院において上部消化管内祝鏡を受け、本研究への参加に同意頂いた患者3名に対しアンケート調査、歯科検診、血液検査、尿素呼気試験を行った。アンケート調査は口腔ケア、生活習慣、食習慣に関する項目を主体にした。歯科検診は当院保健医療フロンティア科学分野にて現在歯数、健全吉敷、歯周ポヶット(PD)アタッチメントロス(AL)、Bleeding on probing(BOP)を測定した。血液検査は抗ヘリコバクター。ピロリ(HP)IgG抗体、ペプシノーゲン(PG)、一般生化学、血液学的検査を施行した。抗HPIgQ抗体陽性かつ尿素呼気試験陽性の者をHP感染陽性とした。胃粘膜萎縮は内視鏡所見から木村・竹本分類を用いて判定した。結果として3例すべてにHP感染を認めた。内視鏡的には胃潰瘍を認めた潰瘍例1名(45歳)、異常のなかった無所見例2名(平均70歳)の2群に分けられた。潰瘍例ではPD最大値8mm、PD平均値2.5mmと、無所見例(PD最大値4mm、PD平均値1.9mm)と比較し高値であった。またBOPを潰瘍例で19%に認められたのに対し無所見例では平均11%であった。内視鏡的な胃粘膜萎縮の程度と歯科的パラメータに関連は認めなかった。アンケート調査の結果では潰瘍例では歯ブラシ以外の口腔ケアのための器具は使用していなかったが、無所見例では2名とも歯間ブラシを使用していた。PDやBOPは歯周病の存在および活動性と関連していることから、歯周病の活動性と胃潰瘍発生が関連している可能性が示唆された。口腔ケアしかし胃癌の発生母地となる胃粘膜萎縮と歯科的パラメータに関連を認めなかったことから、今回の検討では口腔ケアが胃癌を減少させる証拠は得られなかった。今後多数例での報告が待たれる。
