がん看護領域における認定看護師に認定看護師の実践の効果を明らかにするために半構成化面接を行った。本研究は名古屋大学医学部保健学科倫理委員会の承諾を得た。本研究に同意を得られた看護師に対して、電話にて連絡の上、所属施設に訪問。再度研究の主旨を説明して研究の承諾を確認した上で、同意書の署名を得てから面接を開始した。質問内容は、1対象者の属性2認定看護師の役割等である。対象者は同意を得られた、ホスピスケア認定看護師10名について面接を実施し、対象者のは、がん専門特定機能病院3名、総合病院6名、大学病院1名であった。看護師の平均通算経験年数は17.9±4.51年で、認定看護師資格取得後の平均年数は3年あった。面接内容から全1220コード、101サブカテゴリー、30カテゴリーが抽出され、1)ホスピスケアの理念の浸透、2)実践、3)指導、4)相談、5)調整と組織への働きかけ、6)ホスピスケア認定看護師としての役割を遂行するための姿勢6つの枠組で構成した。ホスピスケア認定看護師は、ホスピスの理念を核として、実践、指導、相談活動を行っており、その対象は施設内にと留まらず、地域にまで役割が拡大していた。また、実践、指導、相談の3つの機能だけでなく、調整と組織への働きかけ行っていることが明らかとなった。得られたデータの信頼性と妥当性と確認するために、カテゴリーをもとにアンケートを作成し、量的側面からの調査を実施した。ホスピスケア認定看護師278名と東海北陸地区緩和ケア病棟の看護師を対象にアンケート調査を行った。回収率64.7%であり、因子分析をしたとこと、認定看護師は質的研究と類似した構造になり、一方緩和ケア病棟の看護師は5因子であった。認定看護師は、ホスピスケアの理念を現場の看護師に浸透させていくことで、患者・家族、またケアを提供する看護師のQOLの向上に貢献していることが明らかとなった。
