[背景]認知症高齢者グループホーム(以下,グループホーム)では,利用者家族は利用者の背景と捉える傾向があるが,居宅サービスに位置づけられている以上,利用者本人のみならず,家族をも含めた支援のあり方を検討する必要があろう.そこで,本研究の目的は,グループホームの新規入居者の家族が在宅介護終止をどのように決断し、入居後にどのような心理的プロセスを経過するのか明らかにすることを目的として二ヵ年計画で実施することとした.初年度は,特に近年急増する自己選択により介護役割を担うことが多く、介護にまつわる抑圧の不可視化を招きやすい娘介護者に注目し,娘介護者の担う役割から明らかにすることにより,グループホーム入居前後の家族支援を考察した.本年度は、前年度の結果を踏まえ、さまざまな続柄の要介護者の経験を明らかにすることとした.[方法]本研究の対象はグループホーム入居者のうち,要介護者の介護が必要となった段階から継続して1人の介護者が介護を担当してきた8事例である.介護者に対し,認知症発症からグループホーム入居決定に至るまでの要介護者との関係に関してインタビューを実施した.なお,インタビュー内容は介護者の同意のもと録音し,逐語録を作成・分析した.[結果]認知症症状の発症から入居に至るまで,「I認知症患者の段階的受容」「IIあらゆる資源の利用による介護継続」「III入居後の新たな関係構築」の三段階のプロセスに分けられた.さらに介護者はその基盤に「認知症も病気のひとつ」という認識をもち,介護規範として「在宅介護に対する強い希望」を抱くことが抽出された.本研究は東京大学医学部研究倫理審査を経て実施した.
