项目摘要

本年度は、昨年に引き続き、認知症高齢者グループホーム、介護保険施設等においてインシデント・アクシデント事例を収集し、インシデント・アクシデント発生状況を定量・定性分析によって明らかにした。また、介護事故に関連する要因を利用者の要因(21項目)、介助者の要因(27項目)、システム上・環境上の要因(8項目)の3つの側面から検討した。収集事例総数は、認知症高齢者グループホーム(一部住宅型有料老人ホームを含む)889事例、介護保険施設3817事例合計4706事例である。認知症高齢者グループホームにおける介護事故の種類についてみていくと、最も多く発生しているのは「転倒」で37.3%、次いで「ずり落ち」7.6%、「施設外への無断外出・俳徊」7.0%と続いている。傷害等の状況をみると、最も多いのは「打撲」で7.8%、次いで「内出血・あざ」5.7%、「すり傷」4.7%となっている。発生時間についてみると、8時、14時、18時に発生件数が多くなっており、発生場所では、リビング・ダイニングや利用者自室で多く発生している。転倒事故の約8割が介助行為や見守りをしていない時に発生している。事故発生に関連する要因についてみると、介助者の要因では、「誤薬・誤注」以外のすべての事故で「予測力の不足」が最も多くなっている。システム上・環境上の要因では、「介護職員の連携・共有化不足」「近くにいた職員のサポート不足」が多くなっている。さらに、本年度は、これまでの研究で得られた知見を活用し、本学の学生に対してリスク感性を高めるための介護安全教育を実施した。プログラムの内容は次のとおりである。(1)福祉・介護サービスにおけるリスクとは何か(2)安全管理の基本的考え方(3)介護事故と法的責任(4)ヒューマンエラーのメカニズム(5)事故事例分析と再発防止策(グループワーク)(6)事故防止に向けた組織変革と安全文化の構築。
