本年度の研究は、膝関節痛のある後期高齢女性の生活機能に関与する要因を検討し、後期高齢女性が生活に困難さ・活動制限あり群、困難さまたは活動制限あり群、困難なし・活動制限なし群のいずれに判別されるか、判別分析を行った。対象者は、A県の老人福祉センターを利用している膝関節痛のある在宅後期高齢女性86名。研究方法は、老人福祉センター利用時に、面接聞き取り調査と測定調査を行った。調査・測定内容は、身体機能測定、膝関節痛の程度、日常生活活動の状態、健康度自己評価、日本版膝関節症患者機能評価表等であった。結果は、膝関節痛のある在宅後期高齢女性86名で、平均年齢は79.0歳であった。困難さ・活動制限あり群が18名(20.9%)、困難さまたは活動制限あり群17名(19.8%)、困難なし・活動制限なし群51名(59.3%)であった。3群の平均値を比較すると、困難さ・活動制限あり群と困難なし・活動制限なし群では、年齢(困難さ・活動制限あり群が1.7歳高い)、起立歩行時間(困難さ・活動制限あり群が1.7秒遅い)、最大一歩幅(困難さ・活動制限あり群が15.6cm狭い)、膝関節痛の程度、いずれも有意差があった。判別分析の結果、起立歩行時間、最大一歩幅、膝関節痛の程度、外出のあきらめ頻度、買い物頻度の6項目が3群間の差異をよく表した。判別分析において正しく判別された率は81.4%であった。膝関節痛のある高齢女性が生活に困難さを感じ、日常生活活動を制限しているかを生活動作能力や主観的な疼痛の程度だけでなく、外出の頻度や外出を諦める頻度も評価することによって判別できることがわかった。今後、自立生活を継続するために、高齢者自身及び支援者が生活を評価する指標として用いられるよう、事例数を増やし検証していく必要がある。
