项目摘要

都市部在住の一人暮らし高齢者の健康危機に関するセルフマネジメント上の課題を抽出することを目的とし、本研究では一人暮らし高齢者が自己の健康危機をどのように捉え、個人の危機管理行動について把握した。大阪府下の一人暮らし高齢者4名に対し、日頃感じている健康危機と実施している危機管理について半構成的面接法を実施し、得られた記述データを質的帰納的に分析した。一人暮らし高齢者が日頃感じている危機とは、【緊急時に助けが呼べない状況】、【他者の援助を受けなければならない状況】、【自己判断できず他者に迷惑をかける状況】、【病気や怪我により一人暮らしが維持できなくなる状況】であった。一人暮らし高齢者の危機のリスクとして、【健康状態の悪化】、【認知症の発症】、【歩行機能の低下】、【経済的困窮】、【孤立・孤立死する不安】、【孤立無援状態】、【事故や自然災害の経験】があった。一人暮らし高齢者は自己の健康危機に対して、【食事や運動の予防行動の実施】、【定期受診や服薬の実施】、【家族や近隣者とのつきあいの維持・拡大】、【災害時の備え】、【救急時・緊急時の備え】などのリスク回避行動を実施していた。今後、一人暮らし高齢者とその地域の一人暮らし高齢者の状況に詳しく、支援の経験をもつ地域包括支援センターの保健福祉専門職や住民ボランティアを対象に支援者が把握している一人暮らし高齢者の健康危機の内容と、危機発生時の高齢者への支援や近隣の助け合いの例、当地域の危機に対する支援システム内容について面接調査が必要である。その後、面接調査の結果をもとに、今後、一人暮らし高齢者の健康危機管理を支援するプログラムを作成し、介入研究が必要である。
