项目摘要

目的:本研究では,要介護認定における要支援者の要介護への移行要因を明らかにし,その要因を基に介護予防プログラムを開発することを目的とする。平成19年度:(1)2年後の要介護度の推移の状況とその要因についての分析を行った。(2)18年度までの研究成果をふまえ,閉じこもり・うつ・運動器の機能等について,A市の介護予防事業にて前後比較研究を実施しプログラム評価を行った。報告書を作成し,各市町村へ送付した。対象:(1)大阪府下22市町村の平成16年9-12月の要支援者5,130人(2)A市の護予教室参加者29人方法:(1)-1平成16,17年度の縦断の郵送による質問紙調査(1)-2市町村への対象者の1,2年後の要介護度の調査(2)教室前後(3か月間)の,基本属性,身体心理社会的項目め質問紙調査および運動機能評価結果:(1)全項目の有効回答者1,095人について分析した。2年後の要介護度の悪化を抑制する因子として,老研式活動能力指標得点の1点あがる毎のみが抽出された。促進する因子としては,排泄の失敗あり,外出頻度の週に1回未満が抽出された。(2)転倒歴,老研式活動能力指標得点,認知症早期発見スクリーニング指標得点,うつ,主観的健康感,生きがい,趣味に改善がみられた。考察:(1)介護予防として,要支援者への現存の事業に含まれていない排泄の失敗の予防・支援,閉じこもり予防・支援の重要性が示唆された,(2)介護予防教室への参加は,18年度までの研究成果から導き出された閉じこもり・うつ・運動器の機能等め改善に効果がみられたが,長期的な介護予防の効果の有無を検討する必要があり今後は引き続き半年後,一年後と縦断的に調査する予定である。意義と重要性:介護予防のターゲットである要支援者の要介護度の重度化の関連要因が明らかになり,介護予防が必要な対象を明確化でき介護予防プログラムの開発の示唆が得られた。効果的なプログラム実施により,健康寿命の延伸,経済的な効率,全国に普遍的に適用できる点,より意義があると考えられる。

Objective: This study aims to identify the key factors for the migration of essential support agents, and to develop the key factors for the prevention of migration.
