目的:a.不妊女性が治療開始から治療終結の意思決定に至った過程を明らかにする.b.不妊治療の終結を意思決定した女性と関わった看護者の看護介入を明らかにする.c.aとbを相補的に考察し,不妊女性の治療終結の意思決定過程における看護介入方法を検討すること.方法:不妊治療終結経験のある女性7名,不妊治療終結をした女性の看護経験がある看護者11名に.不妊治療開始から終結意思決定までの事実や考え等を半構成的面接にてデータ収集し,逐語録を分析素材として,個別の内容分析後,女性・看護者別に全体分析を行い,相補的に考察した.結果:女性の治療終結意思決定要因は【医師との信頼関係下の医学的状況の説明と理解】【治療開始・ステップアップ時の整理と終結目安の決定】【年齢と身体的負担の自覚】【経済的・倫理的考察と冷静で客観的な姿勢】【治療継続および妊娠・出産した場合の想橡】【社会通念・追求からの解放と対処能力の向上】【新たな社会的役割の獲得】【ピアとのわかちあいと役割モデルとの出会い】【夫の支えと同意】【治療の遂行感と振り返り】【離婚や養子を迎える選択】.看護者の治療終結意思決定過程における看護は【医学的・客観的情報提供による現状理解の促進】【精神的・経済的査定と全人的な健康支援】【傾聴・共感,同伴する存在としての信頼関係の形成】【治療の節目での目標設定・振り返りと意思の確認・尊重】【夫婦間の関係調整】【気分転換や夢中になれることの勧め】【終結後の生活の想像促進】【終結の自己決定のための情報提供と選択肢の提示】【納得の医療と遂行感の提供】【終結時の振り返り・ねぎらい・自尊感情低下予防】【終結後の健康支援と治療再開への保証】であった.考察:不妊女性自らの意思決定を促進するためには,女性の治療終結決定要因として挙げられ看護者にはなかったピアサポートや家族構成の再構築等への看護を強化する必要がある.不妊治療終結意恩決定過程において,傾聴・情報提供の中で形成された信頼関係を基盤に,適時的意思確認・目標設定・振り返りを経て,終結時の肯定的評価と終結後の健康支援を保証する看護の必要性が示唆された.

Objective:a. To clarify the rational decision-making process from treatment initiation to treatment termination in pregnant women. b. To investigate the complementary nursing intervention methods from rational decision-making process to treatment termination in pregnant women. Methods: 7 women and 11 nurses were enrolled in this study. Start of pregnancy therapy, end of rational decision making, conduct of research, etc. Semi-structured face-to-face analysis, collection of quotations, analysis of individual content, analysis of women and caregivers, overall analysis, complementary research. Results: Rational decision making for women's end of treatment is mainly due to the explanation and understanding of medical conditions under the relationship between doctors and doctors, the arrangement of treatment start, the arrangement of treatment end, the decision of treatment safety, the self-determination of body burden, the examination of ethics, the calm posture of treatment end, the thinking of treatment end, pregnancy, birth occasion, social awareness, the pursuit of liberation, the ability of liberation, the acquisition of new social service, the separation of service end, the consent of husband, the feeling of treatment end, the vibration of divorce, the selection of adopted children. Caregiver's rational decision-making process in the treatment end [medical and customer information provision, promotion of current situation understanding][mental and psychological determination, holistic health support][inclination and empathy][The relationship between husband and wife][The Health support after the end of treatment and re-opening of the guarantee]. To investigate: pregnant women's rational decision-making to promote, women's treatment termination decisions because of the need to strengthen the care of caregivers, such as family structure and reconstruction. During the process of treatment termination intention decision, information is provided, information is formed, relationship is established, meaning is confirmed, goal is set, vibration is returned, positive evaluation is made at termination time, and health support after termination is ensured.
