外来化学療法におけるがん患者及び家族に対する看護師による教育的関わりの現状を把握し、今後の課題を明らかにすることを目的として、郵送法による無記名自記式質問紙調査を行った。対象はがん診療連携拠点病院286施設で外来化学療法に従事する看護師1名とし、病院施設の看護部長に回答者として適切な看護師の選定を依頼した。分析は統計的に行った。結果、216回収され、有効回答数は214(74%)であった。対象者の所属する施設の7割余が総合病院であった。外来化学療法を受ける-日当たり平均患者数は20人未満が8割を占め、看護師の配置は5人未満が9割強であった。認定看護師あるいは専門看護師を配置している施設は約3割であった。教育的関わりを実施しているとの回答は9割を超え、形態としては個別面接が約8割であった。教育的関わりの内容は、「副作用の対処」(98%)「日常生活のセルフケア」(95%)の2項目がほぼ全ての対象施設で行われていたが、「治療法の選択」(22%)や「医師との付き合い方」(30%)は2割程度であった。対象施設の多くが看護師による教育的関わりを行っていることが示されたが、その内容は治療に関する意思決定など心理社会的側面に関する内容については不十分な現状が示された。また総合病院の多くはCNやCNSの配置が困難な状況にあるため、患者家族に提供されるケアの内容や程度に施設間の差があることは否めない。治療を繰り返し受け続けるがん患者とその家族への教育的関わりの内容と方法を今後さらに検討する必要性が示唆された。
