平成12年度の研究において、外来で放射線療法を受けた乳癌患者は、治療開始時には医師の指示通りの治療を違和感なく受け入れていたが、治療が進むにつれ、これからを調節していくのは自分自身であるという意識を持ち、様々な意味でのコントロールを行っていることが明らかとなった。そこで平成13年度の研究では、平成12年度の研究において対象とした8名(乳癌術後で放射線療法終了後の外来通院中)に継続して面接または電話による調査を実施し、各々のセルフイメージやコントロール内容の具体を捉え分析することにより、放射線療法終了後の外来通院中の患者に必要とされる方略的な看護介入を検討することを目的とした。調査内容は、国内外の放射線療法を受けるがん患者に関する研究やセルフイメージ、セルフコントロールに関する文献検討を基に、患者の特性に関する情報、病名・治療に関する受け止め及びそれについての思い、治療への期待、治療後の生活への思いなどとした半構成的面接を実施し、質的な分析を行った。結果、治療後の生活において患者らは、手術を受ける以前の生活に戻っているものがほとんどであり、治療の影響などで日常生活動作上の機能的な不自由さを感じているものはなかった。しかし、通院間隔の延長などで病院との距離を感じており、時に生じてくる再発の不安などにどのように対処したらいいのか、どこに相談をすればいいのかといった、心理的な面でのフォローを必要としていることが明らかとなった。社会的な役割などへ順調に復帰し、さらに新しいセルフイメージを確立していく過程の中で、継続して患者をフォローしていけるような窓口を提供していく看護の実際が必要であるといえる。
