地域における糖尿病療養指導士の育成システム構築の準備として、県内で活動する地域糖尿病療養指導士の抱える課題や、認定後の立場、役割、自己啓発の現状について調査を行った。1.糖尿病療養指導を行う上で抱える課題の明確化研修会で行った療養指導上よく分からないことの質問に自由記載された内容をデータとし、日本糖尿病療養指導士受験ガイドブックに沿って記述内容を分類した。また、療養指導場面での困難性を表す内容に注目して内容分析を行った。結果、記述内容の総数は96であり、糖尿病の概念や診断と治療に関する知識など基本的な内容に疑問点を持ちながら療養指導に当たっていることが明らかになった。また、療養指導場面での困難性には「行動を変化させるアプローチの方法」、「知識不足からくる自信のない対応」、「療養指導と自身の職域との関係」、「効果的な集団教育の展開」があることがわかった。2.糖尿病療養指導士認定後の立場、役割の変化や自己啓発の現状認定者の71%は自分の意志で資格取得を希望しており、97%が今後も更新すると考えていた。職場での周知も広まっており、資格取得による変化として、他のスタッフから意見を求められるようになった、療養指導の中心的立場を任されるようになったという者もいた。また、資格を得ることで自信がついた、責任感がでたとモチベーションの向上につながっていた。反面、取得後の変化なしという者もおり、そういった者は、自身の変化もなしと答え資格更新を躊曙していた。職場異動や時間不足が活動上の問題点となっていたが、資格を有することで研修会への積極的な参加、職場間の連携や他施設で活動する者との交流といった点への関心が高まっていた。全国の資格取得には条件の整わない環境にいる者もおり、地域での認定の位置づけも明らかとなり、認定者の質や意欲維持・向上の面からも育成システムの構築が早急に必要なことが示唆された。
