项目摘要

1、目的経産婦に出産前教育を行った場合、受講者と未受講者の差を明らかにし、プログラムの評価を行う。2、経産婦を対象とした出産前教育プログラム内容(毎第1月曜、2時間、参加者10名程度)(1)自己紹介(2)グループで過去の出産の様子等を自由に語り合う(3)経産婦の出産経過について(4)呼吸法の練習(5)入院の準備について(6)上の子への対応について(7)母乳について、乳房マッサージの練習3、研究方法 1)調査期間:平成18年11月〜平成20年2月2)方法:S病院に通っている妊娠35週程度の経産婦に産前用質問紙(自由記述と選択記述式)、S病院で出産した入院中の経産婦に産後用質問紙(自由記述と選択記述式)を配布。3)倫理的配慮:埼玉県立大学・首都大学東京・S病院の倫理委員会にて承認された。4、結果質問紙回収数は産前271部(84%)、産後115部(43%)であった。うち、有効回答は産前が221名(受講者88名、未受講者133名であった。産後が94名(受講者37名、未受講者57名)であった。1)出産前の結果:質問紙の64項目について受講者と未受講者でU検定を行い、有意差が見られたのは15項目。この結果より、出産前教育を行った場合、経産婦の経過の知識、呼吸法の大切さ、お産による体の変化、経産婦の平均時間、病院につく前にお産になった場合の対処法に関する知識は未受講者と比較して理解度が高かった。また、出産に向けての準備として、呼吸法やリラックス法の練習を行い、前回の嫌だった経験を繰り返さない方法を考えることができていた。2)出産後の結果:質問紙の75項目について受講者と未受講者でU検定を行い、有意差が見られたのは8項目。この結果より、出産を終えた受講者は"妊娠中からの努力をしたい"と思っており、"お産の進行による体の変化"、"平均的な分娩所要時間"、"病院につく前に出産になってしまった場合の対処法"などの知識を持っていた。さらに、実際のお産に関しては"イメ-ジどおりの出産ができた"、"いつも誰かがそばにいてくれた"、"安心してお産に望むことができた"という項目で有意に高かった。また、92.7%の受講者はクラスの受講が役立った、92.9%の受講者がクラスを受講して良かったと解答した。5、結論経産婦であっても出産前教育を行うことで、安心感を得る等の点でよりよい妊娠・出産につながることが示唆された。
