1目的:気管切開の適応と診断された児、家族への看護援助の方向性を得る。2.方法:既に気管切開術を行った児の家族を対象に気管切開の実施を選択するまでの体験について倫理的配慮に留意し半構成的面接を実施した。データは質的帰納的に分析した。3.結果:5人の母親の語りから以下のカテゴリーが抽出された。気管切開の必要性を診断され、実施を意思決定するまでの思い、体験には、【実施するしかないと状況を受け入れる】ようとしつつ【様々な情報源から様々な情報を得ることによっておこる混乱や感情の揺れ】を体験し、【家族なりに子どもの身体的状況を改めて見直し】ながら【気管切開処置によっておこる外観の変化やケアの負担】など処置に対するマイナスの予測ももちつつ、【子どもの将来に対する希望】を見出し【漠然とながらも今後の家族の生活の見通し】をつくりだしながら意思決定するプロセスであった。またこのプロセスは医師、看護師の医療者からの言葉やかかわり、既に気管切開術を行った児や母親から実施後の具体的な児の変化や生活の変容を実際に見たり、聞いたりしながら、現状に対する評価や決定案に対する評価を繰り返しながら意思決定するというプロセスであった。看護職者は、家族が意思決定にいたるまでのプロセスにおいて、単に情報提供するだけでなく、家族の主体的な意思決定プロセスが促進されるように情緒的サポートを行いながら、家族自身が客観的に一つ一つの選択を評価し、修正し最終的な「気管切開処置の実施の有無」を決定できるよう、家族のもつ力を引き出しながら支援していく必要があることが明らかになった。
