今年度は、外来がん化学療法看護に携わる看護師がおよびがん化学療法認定看護師の看護実践内容を抽出するためにインタビュー調査を実施した。対象は、がん専門病院外来化学療法センターに勤務する看護師4名。インタビューは、ケア内容、実践上の困難さなども含めて具体的な事例を挙げてもらいながら30〜40分程度実施した。すでにデータ収集を行った4名の結果は、外来化学療法看護の実践内容の特徴として、《限られた時間》《治療と生活を営む患者》の2つのケア対象者の特徴をもとに、《安全性》《継続性》《専門的知識に基づいたケア》の3つの看護実践が明らかになった。《安全性》では、抗がん剤の取り扱いをする上での確実な投与、一度に多くの患者を観察することの難しさ、ME機器の取り扱いなどが含まれていた。また《継続性》では、限られた外来受診の時間内で繰り返し治療を継続していけるよう患者を支援することの重要性が明らかになった。また外来化学療法を行っている施設の見学や人員の配置状況、勤務体制についても見学し情報収集を行った。2施設の見学を行ったが、治療ベッド数、看護師の配置人数、勤務体制には施設によってかなり異なることが判明した。今後実践モデルを検討する際には、このような施設の用件についての考慮も必要であると考えられた。今後対象施設を1施設追加し、対象者4名選出し、データ収集を行う予定である。施設の勤務体制や概要も含め、分析をすすめ、看護実践内容調査票を作成し、調査を行う予定である。
