本年度は、主として明治末から昭和初期にかけて発行された医学書・医学雑誌・マスメディア資料などを収集・分析し、当時の精神医学者らによって構成された病理学的心理学言説を通時的に考察した。その結果、20世紀初頭には大衆レベルで「神経衰弱」や「神経病」に関する言説が幅広い注目を集めていたこと、また1920〜30年代に入ると、欧米から導入された「精神衛生(Mental Hygiene)」概念が、医学者・福祉事業家などを中心とする専門家の広範な関心を集めていたことを見出した。次にこれら戦前の病理学的心理学言説を考察した結果、そこでは従来指摘されていたような精神医学による「逸脱の医療化」の実践とともに、より軽微な疾患(神経衰弱・ノイローゼ等)を社会的に発見・治療するための「日常生活の医療化」の実践が開始されていたことを実証した。これは現代社会における「メンタルヘルス」概念の起源を考察したものであり、近代日本における精神医学が、全人口の精神的健康を診断する「社会医学としての精神医学」へと変貌した過程を分析したものといえる。さらに以上の考察を行う中で、近代日本の病理学的心理学言説を分析する際に「逸脱の医療化」と「日常生活の医療化」という二系列の言説を包括的に射程する必要性を見出した。すなわち、従来の歴史社会学的研究では前者の実践のみが分析される傾向にあったが、近代日本における病理学的心理学言説を総体的と把握するためには、両者の言説=実践がどのように関連しつつ構成されてきたかに注目する必要がある。たとえば戦前の「精神衛生」概念には、日常的な精神疾患に対処する「日常生活の医療化」という側面と、触法精神障害者や「変質者」に対処する「逸脱の医療化」の側面が並存しており、本年度の研究では20世紀の病理学的心理学言説の原型がこの概念(精神衛生)に象徴されていることを示した。

The origin of the concept of "social medicine" in modern Japan is examined. The mental health of the whole population is diagnosed. The process of social medicine is analyzed. In the course of the above investigation, the necessity of two series of speeches, including "medical treatment of escape" and "medical treatment of daily life", was revealed when psychological analysis of pathology in modern Japan was carried out. In the study of historical sociology, it is necessary to pay attention to the practice and analysis of the former, and to grasp the psychological theory of pathology in modern Japan. The previous concept of "mental health" has two sides: "medicalization of daily life" for daily mental disorders, and "medicalization of escape" for those who violate the law and those who have mental disorders. This year's research shows that this concept (mental health) is characterized by the archetype of psychological discourse on pathology in the 20th century.
