项目摘要

2007年度は、昨年度データ収集を行った自治体病院5病院のデータを用いて経営管理力・競争力を有する病院の病院像について検討を実施した。まず財務面のデータである財務諸表データに関して分析を行ったところ、5病院の中で財務の健全性において大きなバラツキがみられた。しかし、その施設の属性データ(ベッド数や職員数等)や周辺環境のデータ(その地域の医療施設の状況、医療消費量(医療費)、人口規模・構成等)を考慮すると、そのバラツキは小さくなる傾向であった。また、病院のリソースである病院職員の組織文化に関するデータ(A病院559名、B病院442名、C病院535名、D病院802名、E病院167名の計2505名)や、その病院の競争力を示す入院患者の患者満足度のデータ(A病院327名、B病院135名、C病院275名、D病院429名、E病院139名の計1305名)に関して分析を行ったところ、財務データと同様に5病院の中でバラツキがみられた。なお組織文化のデータからは、中間管理職の組織文化が病院全体を代表する傾向がみられたため、経営管理において中間層の評価の検討が必要であると考えられる。次に、「財務状況」、「職員の組織文化」、「患者満足度」の3つの要素間の関連を見たところ、職員の組織文化と患者満足度の間にはわずかながら(正の)相関関係がみられたが、財務状況と組織文化、また財務状況と患者満足度の間には相関関係はみられなかった。以上の結果より、対象病院数が5病院で少ないという限界はあるものの、病院の経営管理力・競争力を評価する際には、総合的な視点から評価・検討する必要性が示唆された。
