项目摘要

長時間労働や変則的な夜勤交代勤務などの過重労働は、労働者の精神的・身体的負担を増長させている。これらにともない発現する著しい心身の疲労感や睡眠障害の慢性化は、精神疾患や循環器疾患の発症要因とされるため看過できない現象である。そこで、本研究では、身体活動、気分・身体症状および認知機能をその場でリアルタイムに長期に測定する簡便な生態学的・経時的手法を用いて、労働負担として現れる慢性疲労兆候を適切に評価しうる指標の確立を目的とした。今年度は、夜勤時に特徴的な気分・身体症状、認知機能と身体活動パターンとの関連について検討を行った。対象者は二交代制勤務看護師17名であった。結果は以下の通りである。(1)身体活動データ(手首部位の加速度のゼロ交差数)を基に算出した睡眠関連指標においては、仮眠休憩を取得する迄の連続勤務時間が長いほど仮眠時の睡眠潜時は有意に長かった(p<0.01)。一方、仮眠時の睡眠時間および睡眠効率では有意な差はみられなかった。(2)勤務時間の経過に伴う気分・身体症状の変化については、仮眠休憩を取得するまでの連続勤務時間が長いほど仮眠休憩前での集中力低下が著しく、仮眠休憩後では持続的注意集中機能がより低下(正答率低下、反応時間遅延、見逃しエラー数増加)し、心身機能の回復度が低い様子が観察された。これらの結果は、長時間夜勤労働の心身機能への影響は、仮眠休憩取得時刻により異なる可能性があり、仮眠休憩取得時刻が勤務時間後半となる場合には、心身機能への負担がより大きくなることを示唆するものであった。夜勤労働負担について生態学的・経時的評価手法に基づき多角的かつ定量的に評価を行った本研究の結果により、心身の不調という主観的な症状に対応する客観的な兆候を、身体活動パターンの分析および身体活動関連指標により明らかに出来る可能性が示唆された。

Long hours of regular night work, heavy labor, mental and physical burdens, increased labor. The symptoms of mental and physical fatigue, chronicity of sleep disorders, mental disorders and circulatory disorders are the main causes of these symptoms. The objective of this study is to establish indicators for the assessment of chronic fatigue in terms of physical activity, body symptoms, and cognitive function. This year, the characteristics of night work, physical symptoms, cognitive function and physical activity are discussed. 17 nurses working for the two volunteers. The result is that the following is not the case. (1)The sleep related index was calculated based on physical activity data (the number of intersections of hand acceleration), and the sleep latency was intentionally longer (p<0.01). The sleep time and sleep rate are different. (2)Overtime on duty accompanied by changes in physical symptoms such as sleep and rest acquisition, prolonged duty, decreased concentration before sleep and rest, decreased attention and concentration after sleep and rest (lower correct response rate, longer reaction time, increased number of escapes), and low recovery of psychosomatic function were observed. The result is that the psychosomatic function of night work is different from that of night work.
