本研究は、妊娠女性を対象として膣・尿路・腸管の細菌叢を前方視的に調査し、若年女性の急性膀胱炎の起因菌として注目されているStaphylococcus saprophyticusの各臓器におけるcolonizationの頻度および、妊娠中の膀胱炎の起因菌としての頻度を明らかにすることを目的として行った。平成20年3月31日までに、筑波大学附属病院を受診した妊娠女性で同意の得られた症例にっいて、妊娠12-16週と妊娠34-36週の2回、尿・膣・直腸の細菌培養・同定検査を行った。尿226検体、膣227検体、直腸227検体において検出されたStaphylococcus属は、Staphylocoecusaureusが尿17検体(7.5%)、膣15検体(6.6%)、直腸3検体(1.3%)、Staphyioeoccus haemolyticusが尿9検体(4.0%)、膣10検体(4.4%)、直腸6検体(2.6%)、Staphylococcus epidermidisが尿14検体(6.2%)、膣24検体(10.6%)、直腸12検体(5.3%)、Staphylococcus homimisが尿1検体(0.4%)、膣7検体(3.1%)、直腸1検体(0.4%)、Staphyloeoecus warneriが尿と直腸でそれぞれ1検体(1.0%)であった。S.saprophyticusは、尿・腔・直腸のいずれの部位においても検出されず、本邦の妊娠女性の膀胱炎の起因菌としてS.saprophyticusの重要性は低いと考えられた。一方、尿からはStaphylococcus属が合計45検体(19.8%)、Enterococcus属が73検体(32.3%)、Escherichiacoh coliが40検体(17.7%)検出され、対象症例において無症候性細菌尿が2例、後期流産が2例、早産が7例に認められた。妊娠中の尿路感染症は早産のリスク因子とされており、本研究で尿検体から分離されたStaphylococcus属あるいはEnterococcus属、E.coliの細菌学的解析は早産の機序解明のりために有用であると期待される。
