Developing a Quality Assessment Scale for Clinical Dysphagia Nursing

Purpose: The purpose of this study is to develop a quality assessment scale (QAS) for dysphagia nursing by both certified nurses (CN) in the dysphagia nursing field and hospital nurses. We seek to clarify the effectivness of CN education and to improve the quality of dysphagia nursing by nurses in hospitals where CNs work.Methods: A QAS was developed based on a literature review of dysphagia nursing and assessed by thirteen experts in dysphagia rehabilitation. A questionnaire survey regarding the QAS for dysphagia nursing was longitudinally conducted among CNs of the dysphagia nursing field registered in 2006 and 2007, and nurses in hospital CN work.Results:1. A QAS for dysphagia nursing by hospital nurses was constructed from sixty-four items and six factors, and Cronbach's a was 0.92 for overall scores.2. A QAS for dysphagia nursing by CNs was constructed from 69 items and 7 factors, and Cronbach's a was 0.98 for overall scores.3. From the results of the Wilcoxson signed-rank test, the mean score of 5 factors (i.e. II ・ III ・ V ・ VI ・ VII) in the QAS of CNs registered in 2006 was significantly increased at one year and seven months, compared with the score at seven months since completion of the regular course.4. From the results of multiple regression analysis, at one year and seven months after completing the regular course, overall QAS scores of nurses in hospitals CNs working showed a statistically significant correlation with the length of dysphagia nursing experience, the length of their nursing experience, and above - average QAS scale scores of CNs.Conclusion: The validity and reliability of the QAS for dysphagia nursing developed were confirmed. The results was suggested ways to improve the educational curriculum for CNs in the dysphagia nursing field.
