本研究は,医療観察法に基づき,入院処遇を受けている精神障害者の家族が,事件や処遇を機にどのような経験をしているのかを明らかにすることを目的とした。平成19年度は,15ケースについて面接調査を実施し,うち13ケースを分析の対象とした。分析はGTAの手法に基づいて行った。逐語録を切片化し,概念名を付与し概念図を作成した。そのうえで各概念図にっいてのストーリーラインを検討した。家族の経験しているプロセスは大きく2つの軸で分けることが可能であった。それらは過去と未来そして現在による時間軸と,社会の中の位置づけに関する軸の2つである。家族はこれら2つの軸の間を揺れ動いていた。家族の語りはさらに「事件前の振り返り」「事件の衝撃」「安心感の獲得」等6つのサブプロセスに分けることができた。事件前の本人の問題行動への対処で疲弊していた家族に,事件発生は多大な衝撃をもたらした。多くの家族が病気や事件の原因の模索を行い,強い自責の念を感じていた。また,家族以外の第三者に被害を及ぼしてしまった事例では,被害者への対応もストレスとして感じられていた。家族は,障害を持った本人が医療観察法による手厚い処遇を受けることにより,安心と希望を取り戻すこととなる。入院処遇中に関係性の見直し,家庭内の雰囲気の変化を経験する家族も多い。その一方で,他者からの心無い言葉に対する不安や,本人の生活に対する不安など,地域生活を送る上での不安は継続的に保持されていた。本研究により,家族支援における介入のポイントが示唆されると同時に,一般の精神保健福祉サービスの見直しの重要性がクローズアップされた。情報提供は重要であるが,混乱の大きい家族にとってそのタイミングと情報量は適切である必要がある。犯罪や精神障害,働いていないこと等に対する社会的イメージが,家族自身の認識に大きな影響を及ぼしていた。
