项目摘要

NBIシステム搭載拡大内視鏡による口腔粘膜疾患の観察を行った。研究期間の症例数は口腔癌症例23例、口腔粘膜疾患症例10例、正常口腔粘膜10例の計43例であった。口腔粘膜疾患の内訳は、口腔白板症や口腔扁平苔癬、アフタ性口内炎などであった。粘膜の観察結果より、上皮乳頭内血管ループ(IPCL)の走行パターンを分類したところ、疾患別に特徴があることが判明した。分類は食道領域で報告されている井上らの分類を口腔粘膜に特化して作成した。正常粘膜ではすべてI型のみに分類され、粘膜疾患はclassII、IIIに分類された。口腔扁平上皮癌ではclassIII、IVに分類された。以上の結果より、それぞれの疾患別にclassI、IIとclassIII、IVとに分けて本分類についての感度、特異度を解析したところ、感度、特異度ともに高い結果が得られた。従って、口腔粘膜疾患に対して本観察を行うことで、口腔癌の早期スクリーニングに有用である可能性が示唆された。

The NBI system is equipped with a large endoscope to detect oral mucosal diseases. During the study period, there were 23 cases of oral cancer, 10 cases of oral mucosal diseases and 10 cases of normal oral mucosa. Oral mucosa disease, oral white board disease, oral lichen planus, oral inflammation The results of mucosal examination, the course of intraepithelial papillary vessels (IPCL), and the characteristics of diseases were identified. The classification of esophageal mucosa is reported in this paper. Normal mucosa is classified as type I, and mucosal disorders are classified as class II and III. Oral squamous cell carcinoma is classified into class III and IV. The above results are based on the sensitivity and specificity of the disease in class I, class II, class III and class IV. The possibility of early detection of oral cancer is also discussed.
