项目摘要

複数の慢性疾患を有する在宅療養者には，そのケアニーズの複雑性から複数の専門職による総合的なケアが必要である．そのため，多職種連携の充実を主軸とした在宅医療介護サービスの質の確保方策が検討され，日本では在宅医療と介護の連携推進事業が多数展開されている．一方，多職種連携を充実させることは，望ましいアウトカムを得るための方策として必ずしも有用でないことが複数の研究で報告されており，多職種連携がどのような場合に利用者のアウトカムの向上により効果的であるのかを詳細に検討した報告や研究はない．そこで，本研究は，多職種連携において中心的な役割を果たす訪問看護師からみた連携に着目し，訪問看護師と訪問看護師以外の専門職との連携しやすさが利用者へのケアにおける目標達成度を高めるのは，目標達成に向けて行うケアがどのような特徴を有する場合であるのかを明らかにすることを目的とした．質問紙調査とインタビュー調査の結果を得た．１)受け持ち訪問看護師が利用者に立案する目標には，【生活支援】を行う目標と【処置観察】を行う目標があった．２)【生活支援】の目標では，受け持ち訪問看護師が連携しやすいと感じているほど，利用者の目標が達成されている傾向がある一方で，【処置観察】の目標では，連携のしやすさと目標達成度に関連がなかった．３)その理由として，【処置観察】の目標には，連携しにくい場合でも訪問看護師のみで対応でき達成される目標があることや，連携しやすい場合でも利用者に避けられない状態の悪化があり達成されなかった目標があることが影響していると考えられた．本研究は，訪問看護師が訪問看護師以外の専門職と連携しやすいことが具体的にどのような場合に目標達成度を高めるのかを明らかにすると共に，訪問看護師によるケア介入方策を具体的に提示するものであり，これらの知見は今後の訪問看護の発展に資するものであると期待された．
