本研究は、小規模多機能型居宅介護事業所の本来の特性である「地域密着性」を回復する手立てとなる「事業所と集住施設との併設・連携モデル」の有用性および設置要件を追究することを目的として、第１に、行政関係者および施設管理者における地域密着性の概念に対する認識状況の把握、第２に、事業所と集住施設の併設・連携事例を対象とする併設効果の検証を行った。第１課題については、事業所の利用圏において地域密着性が希薄である６都市の事業所を対象に、行政関係者および施設管理者への郵送アンケートを通して、地域密着性に対する認識の希薄傾向が顕著であることを立証した。同時に、地域密着性を回復するには、「利用者を事業所の近隣在住高齢者に限定すること」、「事業所が利用高齢者と地域住民との間の地域生活を持続させること」、「事業所が近隣地域の住民組織と組織的に接触・交流を行うこと」の３点が重要な要件となることを明らかにした。第２課題については、当該都市に１０箇所以上の事業所を有する４１都市１，７６６事業所を対象に、「集住施設との併設の有無」、「併設の意図」、「併設の設置形態」、「管理運営状況」等について郵送アンケート調査を行った。その結果、３４都市１５６事業所において集住施設との併設が判明した。併設の意図については、両者の設置時期の関係から、「集住施設先行型（集住施設に居住する高齢居住者の介護強化を図るため事業所を追置）」、「事業所先行型（事業所の利用者確保を図るため集住施設を追置）」、「両施設同時期設置型（両施設の利用者を共に確保することを目的として併設）」の３タイプに分類できることが判明した。併設の効果については、「介護や生活上の効果」が得られたと回答したものが７割強を占め、高い効果が認められた。一方、「経営上の効果」が得られたと回答したものは４割強にとどまり、微妙な結果となった。
