项目摘要

【目的】経口摂取が制限されている子どもにおいて,摂食に関する発達状況を適切に判断し、発達段階に応じた支援を行うために,摂食の発達評価基準を含めた看護介入プログラムを考案・開発する。最終年度は、病気をもつ子どもの摂食に関する発達支援の看護標準化に向けて、開発したプログラム実施のためのプロトコールを作成した。【方法】1.研究デザイン:Case Study2.対象:疾患・治療のために経口摂取が制限されていた乳幼児16名3.介入期間:2～17か月間4.データ収集:これまでに作成・改良した「摂食シート」を用い、介入毎に発達評価・摂食機能評価とアセスメント、介入内容を記述し、それをデータとした。5.データ分析:得られたデータをケース毎に看護過程の手法により、介入方法の適正と効果を分析、各ケースの共通点、相違点を比較検討した。6.倫理的配慮:研究者所属機関の倫理委員会の承認を得て、すべてその手続きに従った。【結果】対象患児は、短腸症候群3名、先天性食道閉鎖症3名、生体肝移植後2名、先天性心疾患7名で、胸部リンパ管腫1名で、介入時の年齢は7ヶ月～6歳8ヶ月であった。対象患児は、疾患や治療のために摂食に関する学習不足によって摂食機能の発達途上であり、他の発達とのアンバランスがあったが、アセスメントに基づいた摂食の直接・間接訓練、発達訓練などの介入の結果、全員に摂食機能・行動の発達的向上が認められた。これらデータをもとに、アセスメントからプランニングまでのプロトコールを作成、図式化を試みた。このプロトコールでは、「摂食シート」を用いて、発達評価・摂食機能評価・介助法の評価により、介入段階で摂食に関するnegative factorを特定すると同時に患児の強みに着目すること、それらに特化した間接訓練・直接訓練・発達訓練を取り入れることができるようにフローチャートにより表現した。今後は、これらプロトコールの検証を進めるべく、介入事例を増やすこと、看護師による本プロトコールの実践モデルを構築していくことが課題である。
