项目摘要

平成２４年度の実施計画では以下の３点を目標とした。（１）円皮鍼による刺激が嚥下反射を改善するという研究結果を報告する。（２）円皮鍼を貼付し続けることで誤嚥性肺炎の予防ができるかどうかを検討する。（３）鍼刺激が嚥下反射の改善に寄与するメカニズムを探る。（１）に関しては、論文を作成し現在投稿中である。（２）の研究計画は、嚥下機能の低下している症例を、嚥下反射を改善する経穴に円皮鍼を貼付する群と貼付しない群に分けた。円皮鍼を貼付する群には連日医療従事者による貼り替えを行い、３か月、６か月、１２か月後の各群の嚥下反射と期間中の肺炎発症率・発熱日数・栄養状態の変化等を比較した。これまでに収集したデータの比較では、両群間の肺炎発症率・発熱日数・栄養状態の変化には有意な差は認められていない。両群間を十分に比較するにはさらにデータの数を増やすことが必要だが、これまでのところ円皮鍼を長期に貼付し続けることが誤嚥性肺炎の予防に寄与することは証明できなかった。（３）の研究計画は、鍼刺激が嚥下反射の改善に寄与するメカニズムを探るために、鍼刺激に対する頚部動脈の血流変化を高性能超音波診断装置にて測定するものであった。具体的には、６５歳以上の健常者と脳血管障害を持つ症例との二群に分け、嚥下反射を改善する経穴への鍼刺激により血圧、脈拍、内頸・外頸動脈の血流量等の変化を測定する。高性能超音波診断装置はベッドサイドに運搬することができないため、測定を行うには研究室まで協力者に来訪してもらわなければならない。脳血管障害を持つ高齢者は移動困難な方も多く、十分な数の協力者を集めるまでに至らなかった。
