市町村が介護保険法に基づいて行う介護予防事業の中でも特に委託率が高く，参加者数も多い運動による介護予防事業について，次の①②を実施した。①業務委託の現状と課題を量的に明らかにする。A県58市区町村の事業担当保健師各1名と，通所介護事業所など5種類の事業者数に応じ，層化して無作為抽出した1,000事業者の運動指導者各1名を対象に，郵送自記式質問紙調査を行った。1社当たりの受託数は1～14自治体と差があり，資格は健康運動指導士，看護師，介護福祉士の順に多かった。事業担当年数は保健師2.7±2.0年，運動指導者が4.4±2.9年で有意に長かった。運動指導者は契約内容に合意していても，委託を受ける弱い立場を感じていた。またモニタリング結果の共有状況の認識に保健師と運動指導者に有意差があり，途中経過での情報共有への必要性が示唆された。成果を感じる項目にも違いがあり，運動指導者は参加者個別の変化を重視する傾向があった。事業者間の力量差は保健師も運動指導者も約半数が感じており，その差へのアプローチが必要である事も互いに認識していた。②委託事業者と協働関係を発展させながらサービスの質の向上を目指す保健師に求められるマネジメント能力を質的に明らかにする。運動指導者3名（鍼灸マッサージ師1名，健康運動指導士2名）と，保健師3名に半構成化面接を実施した。運動指導者が保健師に求めるマネジメント能力は【人的・物的資源に精通し，調整できる】，【行政組織・地域・民間資源の整理ができる】，【ニーズのある住民を掘り起こせる】，【自治体のビジョンを示し，共有できる】ことであった。保健師は常に地域づくりやライフサイクルを通じた支援につなげる意図を持ち，【事業者と意思疎通】をして情報・安全管理をし，行政組織内外の人間関係に配慮しながら【マンパワーの配分】をし，【人や組織をつなぐ】能力を重視していた。
