注意欠陥多動性障害および自閉症は中核症状が異なるが、併存率は10～20％にのぼる。早期介入のためにも同障害の鑑別は極めて重要である。本研究はADHDおよびASDの鑑別に貢献するために、実行機能仮説に基づき、詳細な実行機能評価が可能なバッテリーであるCANTAB施行中の脳活動を近赤外線分光法(NIRS)で測定し、ADHDおよびASDの違いを検討した。本研究の結果は国内および国際学会、国際学術雑誌などで報告した。以下、その詳細である。CANTAB課題の視空間性ワーキングメモリ(SWM)課題において、定型発達(TD)群と比較して、ADHD群の得点が有意に低下していた。一方NIRSで測定した脳活動変化については、視空間性短気記憶課題にいて、TD群に比べ、ASD群において前頭極部位のチャンネルで有意な脳活動の低下を示した。本研究の結果から、ADHDにおける視空間性ワーキングメモリの弱さが示唆され、先行研究と符号する結果が得られた。今回の結果ではADHD群とASD群では有意差は検出されなかったが、ADHDではASDに比べて視空間性ワーキングメモリー障害の程度が重たい可能性が示唆された。一方、NIRS波形ではASD群においてTD群とADHD群とは異なる脳活動パターンであることが示唆された。加えてわれわれは、ADHD群で中核症状と密接に関係する視空間性ワーキングメモリの観点からMPH治療効果をCANTABとNIRSを用いて検証した。未服薬日と服薬日を比較した結果、課題成績では治療効果を認めなかった。一方、同時に計測した脳活動では課題難易度に応じた脳活動変化で服薬条件と未服薬条件で有為な差を認めた。視空間性ワーキングメモリ課題中のNIRS測定が、簡易で客観的なMPH治療効果判定になることが期待された。

Note that hyperactivity disorder and autism have different nuclear symptoms, and the coexistence rate is 10~20%. Early intervention is extremely important in identifying common hazards. This study discusses the contribution to the identification of ADHD and ASD, as well as the detailed functional evaluation of ADHD and ASD activity during CANTAB implementation by near infrared spectroscopy (NIRS). The results of this study were reported to the national and international academic societies. The following is a detailed description. The visual spatial distribution of CANTAB subjects (SWM) is lower than that of TD subjects and ADHD subjects. The activity of NIRS was measured in different ways, such as spatial memory task, TD group, ASD group, and intentional activity decrease. The results of this study include: 1. ADHD; 2.
