项目摘要

実施した研究成果として、調査計画に基づき以下に報告する。CHF患者のヘルスリテラシーの様相として情報読解力，情報理解力，意思決定力，患者役割遂行力に関する記述を抽出し，新たにCHFと診断された男性患者のHLの様相について共通具体的していた点は以下のとおりであった。CHF患者の情報読解力は，心不全の病態，薬物療法における作用，運動療法や食事療法など心不全に関する医療情報を読むことや聞くことは十分にできていた。しかし，心不全症状の発生機序に関しては読み解くことは不十分であった。CHF患者の情報理解力は，自分自身の身体に心不全症状の出現や増悪を認めた場合には，獲得した心不全に関する情報と症状を結び付けて理解できるが，症状の経験がない場合は，病態と十分に結びつけて理解することは困難であった。さらに意思決定力に関して，CHF患者は日常療養の中において経験的に浮腫などの心不全症状を捉えていたが，症状の程度の評価や受療の必要性などの判断に情報を十分に活用できる能力は不十分であった。患者役割遂行力においてCHF患者は，心不全の病勢や症状に疑問や不安などを持っている場合においても，疾患や治療，今後の療養に関することは医師や看護師などの医療者に委ねるとする依存的状況であり，意図的な医療者へのコミュニケーションをとる能力に欠けていた。しかし，苦痛な心不全症状を経験している患者の場合には，心不全の薬物療養や運動療法などの疾患管理においては自ら情報を集め，自分自身に適切に健康情報を活用して自らの療養管理に反映させる行動をとっており，患者役割遂行力は個人差が見られた。現在、以上の結果に基づき、循環器内科及び循環器内科入院病棟を有する総合病院で、CHF患者を対象とした患者指導の看護実践を行う中堅看護師を対象にインタビューガイドを用いた半構成的面接の実施を開始している。
